Cigna's commercial, employer-sponsored, and Medicare Advantage plans include behavioral health benefits administered through Evernorth Behavioral Health. Cigna's published medical coverage policy recognizes Transcranial Magnetic Stimulation as medically necessary for adults with treatment-resistant major depressive disorder who meet defined criteria, and the policy has been updated to reflect FDA clearances for OCD and anxious depression as well. The 2024 CMS National Coverage Determination for TMS pulled Cigna's commercial book of business into closer alignment with Medicare's coverage floor.
What Cigna typically covers
- Adult psychiatry and medication management — evaluations, follow-ups, prescription oversight
- TMS therapy for FDA-cleared indications — MDD/treatment-resistant depression
- Outpatient psychotherapy — individual sessions with in-network behavioral health providers
Cigna's Open Access Plus, HMO, EPO, and PPO plans have different in-network rules and copay structures. Some employer-sponsored plans are self-funded under ERISA, which can override Cigna's standard medical policy.
Prior authorization for TMS
A standard TMS course runs 36 sessions. Cigna routes TMS authorization through Evernorth Behavioral Health and typically requires:
- DSM-5 diagnosis of major depressive disorder
- Documented failure of at least two antidepressants at therapeutic dose and duration from at least two pharmacologic classes
- Baseline depression severity score (PHQ-9, HAM-D, BDI, or equivalent)
- Documented psychotherapy trial or clinical justification for omission
- No contraindications to magnetic stimulation
Our clinical team prepares and submits the prior-authorization packet to Evernorth and tracks the determination, which typically arrives within 5 to 14 business days. Cigna may also require interim severity re-scoring at session 10 or 20 to authorize the remainder of the course.

Cigna's commercial plans vary considerably by employer rider, especially in self-funded ASO arrangements where the employer sets the benefit. The only reliable way to confirm your TMS coverage, copay, and any session cap is a verification against your member ID.
