TRICARE coverage details
TRICARE is the Department of Defense health program covering active-duty service members, retirees, National Guard and Reserve members, and their families. In California, TRICARE West is administered by TriWest Healthcare Alliance, with most beneficiaries enrolled in TRICARE Prime or TRICARE Select. TRICARE has formally covered Transcranial Magnetic Stimulation for treatment-resistant major depressive disorder in adults since 2016, and coverage criteria are codified in the TRICARE Policy Manual. The 2024 CMS National Coverage Determination for TMS broadly reinforced federal alignment on TMS as standard psychiatric care.
What TRICARE typically covers
- Adult psychiatry and medication management — evaluations, follow-ups, prescription oversight
- TMS therapy for FDA-cleared MDD/treatment-resistant depression — covered under the TRICARE Policy Manual for beneficiaries meeting defined criteria
- Outpatient psychotherapy — individual sessions with TRICARE-authorized providers
Active-duty service members generally require a referral from their primary care manager or military treatment facility before accessing civilian outpatient psychiatric care, including TMS. TRICARE Prime retirees typically require a referral for specialty care; TRICARE Select beneficiaries do not.
Prior authorization for TMS
A standard TMS course runs 36 sessions. TRICARE requires prior authorization for TMS through the regional contractor (TriWest in the West region). Documentation typically required:
- DSM-5 diagnosis of major depressive disorder, severe
- Failure of at least two antidepressant trials at therapeutic dose and duration from at least two pharmacologic classes
- Baseline severity score (PHQ-9, HAM-D, or equivalent)
- Documented psychotherapy trial
- No contraindications to magnetic stimulation
- Treatment delivered by an FDA-cleared TMS device by a TRICARE-authorized provider
